For the past two decades, companies marketing insulins recommended initiating insulin early on in order to attain a target A1c goal of 7%. Today, experts say too low of an A1c can do more harm than good in elderly patients with Type 2 diabetes. The 2020 American Diabetes Association (ADA) guidelines now recommends an A1C 7.5 % – 8.5 % in seniors, those with multiple chronic conditions and people with cognitive impairments.
